Т.П. Авраменко ДЕРЖАВНЕ УПРАВЛІННЯ СИСТЕМОЮ ОХОРОНИ ЗДОРОВ’Я В УМОВАХ СИСТЕМНИХ ЗМІН В УКРАЇНІ: СУЧАСНИЙ СТАН, ПРОБЛЕМИ ТА ПЕРСПЕКТИВИ РОЗВИТКУ
У статті висвітлено проблемні питання державного управління сфери охорони здоров’я в умовах системних змін в Україні. Здійснено аналіз сучасного стану вітчизняної медичної галузі й визначено основні її проблеми. Розкрито основні причини невдалого досвіду попередніх реформ сфери охорони здоров’я в Україні. Обґрунтовано перспективні напрями реформування сфери охорони здоров’я в Україні, зокрема фінансово-економічний, організаційно-управлінський, нормативно-правовий і механізми оптимізації діяльності громадської охорони здоров’я.

В статье определены проблемные вопросы государственного управления здравоохранением в условиях системных изменений в Украине. Проведен анализ нынешнего состояния отечественной медицинской отрасли и определены основные ее проблемы. Раскрыты основные причины неудачного опыта предыдущих реформ здравоохранения в Украине. Обоснованы перспективные направления реформирования здравоохранения в Украине, в частности финансово-экономический, организационно-управленческий, нормативно-правовой и механизмы оптимизации деятельности общественного здравоохранения.

The article identified public administration of health care in a systematic change in Ukraine. The analysis of the current state of the domestic medical industry and identifies its main problem. The main reasons for unsuccessful experience of the previous reforms of health care in Ukraine are opened. The promising areas of healthcare reform in Ukraine, including the financial and economic, organizational, administrative, regulatory mechanisms and optimization of public health. The healthcare system in Ukraine is characterized by deep problems that require key state solutions in this area: inadequacy of the financing instruments of the healthcare system and irrational use of existing resources; inefficiency of organizational and functional structure of the healthcare system; orientation of the Ukrainian healthcare system on morbidity rather than preventive healthcare. Besides, the Ukrainian healthcare system faces a range of unsettled challenges (employment issue, quality and availability of healthcare services etc.) and the key challenge is an inadequacy of financial and economic tools to guarantee the healthcare system operation. Andtotal healthcare costs increase due to finances of ordinary people and it deals nothing with improvement of healthcare services. Social and economic changes in our country require immediate efficient measures in reforming of state management of the healthcare system. For this purpose the following actions should be taken:
– To strengthen the healthcare financial basis: to implement regional monitoring of health conditions of the population to justify state costs and provide their efficient spending; to create framework for gradual transition to multiple (from different sources), budget and assurance healthcare model etc.
– To improve organizational and legal framework for the healthcare system that affect financial situation of the healthcare system: to provide personnel; to establish contractual framework between the state authorities and institutional medical centres regarding rational use of their resources for the entire community; to create basis of medical standards and clinical protocols on medical aid provision on evidence based medicine etc.
